The information we have about Radzinsky was provided by Kelvin Inman, in conversations with Desmond. Radzinsky was first mentioned in "Live Together, Die Alone".

The information is of uncertain accuracy as Kelvin proved he was not someone to be trusted (with lying to Desmond about his true activities outside the Swan).

According to Kelvin Radzinsky was his original partner in the Swan, and he was the one who started the blast door map. Kelvin also claims Radzinsky had photographic memory (which helped him with the map); discovered how to fake a lockdown and selectively edited the Swan Orientation Film.

Kelvin claimed Radzinsky committed suicide by putting a shotgun in his mouth and firing it, causing a blood stain on the ceiling.
Radzinsky's death is confirmed by Gregg Nations at The Fuselage, who says he is also not "Patchy."

Inman said to Desmond after Radzinsky died, the worst thing to do was to bury him within 108 minutes. If Inman was telling the truth, this must mean Radzinsky must be buried close to the Swan.

Noppers. We just know he was Kelvin's partner, started the blast door map and killed himself.

Well, unless you count the shot of the blood stain as a pic.
